I've never done a mail art swap before...do we mail the decorated envelope empty or do we put something in it?

@lynnpinkdaisy when the envelope is the object of the swap and no mention is made of including anything inside then no you don't have to. I often make a small note of 'empty' on an envelope in that case

Yes. I forgot to add that you do not need to include anything unless you want. Just be aware that your partner may not reciprocate.
